April 2012 Midwifery Board Exam Room Assignments for Tuguegarao

The Professional Regulation Commission released online the April 2012 Midwifery Board Exam Room Assignments for Tuguegarao.

The Midwifery Licensure Exam will be happening on April 15 and 16 2012 in Bagay Elementary School in Tuguegarao City, and in all PRC regional centers across the country.

Instructions to all takers:
1. Board Exam takers should report before 6:30 in the morning every examination day. LATE EXAMINEES WILL NOT BE ADMITTED.
2. Bring the following: Notice of Admission (NOA), Official Receipt, pencils no. 1 or 2, black ball pens, metered-stamped window mailing envelope, long brown and long plastic envelopes.
3. CELLULAR PHONES AND OTHER ELECTRONIC GADGETS ARE STRICTLY PROHIBITED.
4. Wear the prescribed dress code.

Reminders to all the examinees your are advised to use the same name in all examination forms during the said examination. If you found any error in name, date of birth or application number, kindly report it to the application division before the exam date or request the room watchers to correct it on the first day of examination.
